Summary:

Independently provides comprehensive quality medical services in a compassionate manner to patients to include for example, complete physical examinations, diagnosis, treatment, prevention of both chronic and episodic disorders, preventive care, immunizations, primary care and treatment, health education, prescribe medications, order and interpret lab testing and other diagnostic test and screening, vision/hearing, women’s health, family planning and nutrition and health education.

DUTIES AND RESPONSIBILITIES:
  1. Responsible for obtaining patients’ medical histories.
  2. Responsible for each patient medical care from discovery of the disorder to recovery.
  3. Responsible for performing complete physical examinations, including ordering, interpreting and evaluating diagnostic tests and examinations.
  4. Responsible for diagnoses and treatment of both chronic and episodic disorders including complications of otherwise normal processes such as e.g. patients CHDP or childhood development.
  5. Responsible for establishing and documenting the patients health care plan and prognosis, maintaining records of each case that are sufficiently complete for any physician reviewing them to evaluate the effectiveness of the plan and assists patients with setting their self-health care goals.
  6. Responsible for initiating drug therapy, taking responsibility for such therapy when performed prior to consultation with a physician.
  7. Responsible for determining when the patient has recovered from the disorder and releases the patient.
  8. Responsible for providing health education and instruction in the areas of e.g., health/sickness, personal hygiene, nutrition, sexual behaviors, and medication usage as applicable.
  9. Responsible for meeting and consulting with other staff providers on an on-going, regular basis to discuss clinical concerns and opportunities for care improvement. Additionally, consults with physician on duty as necessary and refer patients to appropriate resources.
  10. Responsible for ensuring patient/client confidentiality; demonstrates complete discretion when discussing patient information.
  11. Responsible for providing medical services that meet standards of care, and agency policy, procedures, and protocols.
  12. Responsible for participating in The Joint Commission (TJC) PCMH daily morning huddles.
  13. Responsible for performing patient triage to include direct patient assessment, patient treatment and, consults with the medical team throughout triage and refers patients to the medical team for treatment as needed.
  14. Responsible for actively participating in the agency’s Improving Organization Performance Program to include e.g., Infection Control, Safety and Emergency Preparedness Programs and the TJC.
REQUIREMENTS:
  1. Current California Registered Nurse license
  2. Masters Degree from an approved accredited University School of Nursing (completion of a program of training which conforms to standards established by the California Board of Registered Nurses for Nurse Practitioners
  3. Current Nurse Practitioner Certification and two years of Nurse Practitioner experience
  4. Current board certification
  5. Current DEA certificate
  6. Current CPR certificate
  7. Bilingual (English/Spanish)
  8. Valid California Driver’s license and California basic automobile insurance
  9. Knowledge of electronic medical records
  10. Ability to communicate effectively both verbal and written communication
  11. Ability to work well in a professional team oriented environment
